Caption : We are at this awful place because of politics. There is no abortion law in Canada, and so women are not bleeding to death or dying from sepsis in parking lots. Wonder why we don’t have affordable contraception, better studies for menopause, or even a basic understanding of endometriosis? Wonder why it’s so hard to find health care professionals in many places? The root of so many issues is related to politics. While it is clear that many previous governments could have done better by women in many ways, we have one party that is actively trying to harm women. And if the GOP is elected, we can almost certainly expect a national abortion ban. And if a national abortion ban won’t affect you, or you think it won’t because you may one day have a daughter or daughter in law with ruptured membranes and an infection at 19 weeks, remember this: in the United States abortion is the canary in the coal mine. Contraception and IVF will be next. There are many dark turns this could take. And for those who are still not swayed. Pharmaceutical companies will almost certainly stop testing new drugs on those capable of getting pregnant, because a miscarriage could be viewed as an abortion. Caption : This dude is such an example of misogyny in the world of “alternative medicine.” The main cause of primary dysmenorrhea (“regular” painful periods) is prostaglandins. Denying this means you want to hide something from women and girls, or are just too uneducated to know. There are of course other causes of painful periods, such as endometriosis and adenomyosis, and these are referred to as secondary dysmenorrhea. There are evidence based treatments, and when someone asks for therapies that aren’t pills, the first thing to do is to explain that NSAIDs can be highly effective for primary dysmenorrhea. The person asking the question might not know! An ethical person would explain that the vitamins and herbal remedies are poorly tested compared to medications and may not contain what they claim. In addition, hormonal contraception of all kinds can be very effective for primary dysmenorrhea. The best evidence for non medication alternatives for painful periods seems to be a TENS unit. I have a whole chapter on alternative approaches for painful periods in my book Blood. It isn’t wrong to choose these methods, it is wrong for anyone to misinform about them. When you present options that are unstudied or understudied it is important people know that so they truly have informed choice. And don’t tell a teen or woman that her painful periods are due to stress. Just don’t. It wasn’t stress that kept me home from school one day each cycle, it was my effing prostaglandins. Then again, maybe chiropractors and naturopaths don’t actually learn about prostaglandins in school, which wouldn’t surprise me.Likes : 9358

Caption : A huge red flag for me is people claiming that there is a post birth control pill syndrome. This has never been described in the medical literature, so this rapid development of supplements, books, and courses about it is alarming. You can’t claim something is understudied and then turn around and designed untested therapies for profit. I mean, you can, but then you are a hypocrite. When you stop the pill the hormones are out of your system very quickly, often within 7 days. Fertility returns very quickly as well. And there are no robust studies showing any link between the pill and nutritional deficiencies. While it is true that there are aspects of how people feel on the pill that have been understudied, you don’t help that by selling unstudied supplements or courses, selling unstudied therapies is profiting from gaps in the system, not closing them. The birth control pill often treats many conditions, for example acne and PMS. It is even possible that people start the pill before those conditions have emerged, for example maybe they had very heavy periods at age 13. And so when they stop, they experience these conditions because they are no longer being treated by the pill. If you want to stop the pill, just stop. You do not need unstudied supplements or coaching for a condition that has yet to be described in the medical literature. Caption : Just my regular reminder that no supplement has quality data to recommend it for symptoms of menopause. These kinds of supplements are money making machines. Think millions of dollars. In my opinion it’s bad enough that these products exist, but when providers sell these kinds of supplements, it tells me they are unconcerned with science and your health and are more interested in their bottom line. There are doctors and naturopaths with their own speciality products, and yet you have no way of finding out how much money is involved. You can look up doctors to see what they from drug companies, but you have no way of knowing with supplements or with non physicians. And of course there are people who don’t have their own line of supplements, but promote specific products. You have no way of knowing if money has changed hands. Caption : The paper I am referencing is “Medical conspiracy theories and health behaviors in the United States” JAMA 2014;174:817-818 The guidelines I am talking about here are the 2022 Menopause Society hormone therapy guidelines. I don’t know of any randomized clinical trial that has been published since these guidelines came out that refutes them. And as I have mentioned before, the dementia data was covered in detail at the 2024 Menopause Society meeting and the conclusion is that they stand. Wanting a hypothesis to be true isn’t science, no matter how much it supports your business model! Caption : Some people need to look up the words “dismissive” and “gaslighting.” Ahem. Explaining the state of what evidence based medicine tells us about MHT is not being dismissive or gaslighting. I mean, come on. Saying women should use MHT (if they want to and it is safe for them and tolerated) for symptoms of menopause is dismissive how? Taps mic, menopause symptoms are not only hot flashes or night sweats. Explaining that MHT can be good for the bones, but not everyone will need it (and providing tools to help you sort that out, in the podcast) is not being dismissive FFS. Also there are non estrogen therapies for the bones too and weight bearing and resistance training also important (always need that plug). And explaining that evidence based medicine doesn’t currently support MHT for protecting the heart or the brain is not gaslighting. I really hate when people throw that term around. Might all this change? Sure! That’s science. But I have said many times before and Professor Susan Davis said it as well, if the evidence were clear it would be in the guidelines. The last Menopause Society guidelines are from 2022 and the international menopause Society has a fantastic tool kit that was published in 2023. It is othering people and creating a mythical past when we were all healthy. There are so many parallels between fascism and wellness, so I was not surprised at all with Oz’s choice of words. Caption : I made the mistake of accidentally googling myself… Calling out people who sell unregulated, untested pharmaceuticals (supplements with health claims, not products like prenatal vitamins or iron for iron deficiency) seems to get people mad. Like really mad. I’ve been doing this for ages (hello GOOP) and have been pretty vocal about how these people are making money hand over fist for products that are not recommended in guidelines and often have little to no (it is usually no) data to support their use. Remember, if the data were good they would be in the guidelines! Whether it is a company, celebrity, medical doctor, naturopath, chiropractor, PhD, nurse…ok you get the picture…doesn’t matter. It irks me to no end that people give their heard earned money for false promises…false promises that may also net them liver injury to boot. So why now all of a sudden people are hot about it is fascinating to me. I’ve called out numerous prenatal vitamins, the liver shield, menopause supplements, probiotics etc. I have some theories, and I’m sure I will write about it at some point. Thinking I’m mean, the person not making money from supplements, for pointing out the dismal research, safety issues, and overall grift is a weird flex. No one likes to be led astray. But really, shouldn’t the anger be directed at the people making tens or hundreds of thousands or even millions of dollars from selling products with dismal data? Being appropriately confident is just that. And I’m not going to lie to you to oversell things for attention. Women’s health has too long been a pendulum where something is new and amazing or essential and pushed on everyone or it’s the opposite. I want to help women with data and, when that is absent, experience, not trends. Because that is the kind of care that I want. I’m not looking for anything in reply. But if you are hate following me, maybe reconsider. Life is too short for that. And please don’t go looking for the sub-Reddit. Here we are talking about iron deficiency, which it would be fair to call an epidemic, especially for women under the age of 25 (but to be sure we see it among all ages of women who menstruate!). Iron deficiency is its own medical condition. Anemia is just one consequence! Brain fog, hair loss, restless leg syndrome, and depression can all be a consequence of iron deficiency. Caption : If you plan on living to age 85, you are a 50% chance of getting shingles, which can be very painful and also cause stroke and inflammation of the spinal cord, among other complications. In addition, about 5-15% of people who get shingles will develop post herpetic neuralgia, which can be a very painful condition. Enter Shingrix, the newer shingles vaccine which offers excellent protection (reduced the risk of shingles by 97% vs placebo). It also has long lasting immunity (10 years and counting), and is as effective for people over 70 as it is for people who are 50. Caption : I wrote about an interesting new study published this week in the New England Journal of Medicine, about treating male partners to reduce the risk of recurrent bacterial vaginosis. There’s actually been a fair amount of research trying to solve the puzzle of bacterial vaginosis. We know that it is a dysbiosis, and beneficial lactic acid producing lactobacilli ate replaced with anaerobic bacteria. We also know that biofilms, which are communities of microorganisms that adhere to surfaces are also important. While we know that sexual activity is associated with bacterial vaginosis, we don’t have an understanding of how this happens in the same way that we understand how chlamydia or HIV are transmitted. Previous studies treating male partners with oral antibiotics have been disappointing. This study takes a new approach, looking at both oral and topical antibiotics for men. The results are impressive. When women took antibiotics for bacterial vaginosis and their male partners were not treated their risk of recurrence within 12 weeks was 63%, but when their male partners were treated, the risk of recurrence was 35%. So many supplements, so little good science to support them. For example, his expert collection at momentous includes Fadogia Agrestis for hormone support, but hormone support is a medically meaningless term. There are no studies on the page supporting its use, but a claim is made that it can raise levels of LH and increase testosterone. I found one study in PubMed from 2005 showing Fadogia agrestis increased testosterone levels over one week…in male rats. Since the question is about the FDA, that is the best place to look. In addition to a statement from the FDA indicating mifepristone was approved in 2000, in the search you can see also the package insert, which would only exist if the medication is FDA approved. Swipe to see the video of my quick search for “Is mifepristone FDA approved.” Numerous reputable sites came up, like the FDA, ACOG, and KFF to name a few. Since the question is about the FDA, that is the best place to look. In addition to a statement from the FDA indicating mifepristone was approved in 2000, in the search you can see also the package insert, which would only exist if the medication is FDA approved. Caption : I wrote about all the problems with turmeric and curcumin for my latest post for The Vajenda and how novelty data recommends it for anything in menopause. Special thanks to @kcklatt for fielding my questions! Did you know that despite more than $150 million in NIH funds we still don’t have good data supporting a formulation that does anything? One of the biggest researchers into turmeric/curcumin has had 30 papers retracted for scientific misconduct and many more are in question. Also, many papers on turmeric/curcumin come from paper mills. There are now turmeric and curcumin supplements that are formulated to improve absorption/bioavailability, but we have little data about their safety and efficacy and many countries have reported liver injury from these products. Despite the lack of quality data supporting its use, the global turmeric/curcumin market is $70 million a year and is estimated to be $112.5 million in 2025. It really does seem that turmeric is the golden spice! It’s no wonder doctors sell it. As I showed you in a previous piece, one just has to sell 8333 bottles at $35 each to net $195k. Women deserve science, not this kind of dismal data. It’s enraging to think supplement companies don’t have up do any research because the NIH will find these studies. And did you know people writing articles on turmeric and curcumin still reference the researcher who has all those retractions? It’s maddening.Likes : 4612
