A couple of related thoughts. The first, from the incredible insightful Carl Sagan. I used this quote at the end of my talk at the @festivalofdangerousideas It think of this a lot when people attack me or get mad when I focus on evidence and the scientific method. Sunk cost fallacy is so human. The second is a very insightful comment from @hankgreen about vibes over facts. It seems to me that in women’s health there is a lot of this. I’m sure there is an essay in there somewhere…

Tasmania is truly a special place! I have always wanted to visit, and now all I can think of is how to get back!! The beaches and countryside are amazing. The first shot is Wineglass Bay. We saw Tasmanian Devils, which are the most curious creatures! They are the black ones in the photos. Incredible bite strength. They make the most spectacular growling/howling noise when faced with another devil. I also got to cuddle a baby wombat. They apparently need intense cuddling for the first two years of their life. And the final photo is a Quoll, which is also known as a baby faced assassin. You want to cuddle it, but it wants your finger! Also, had some fantastic local beer and wine. Perfect place for a honeymoon!

Another update from the Menopause Society Meeting The 2022 Guidelines hold, menopause hormone therapy is not indicated for prevention of dementia. The session that covered this was fantastic! I know there are accounts out there that scare you and tell you that without MHT you will get dementia, but that is not supported by the literature. And if their answer is it is inadequately studied…well, they can’t draw conclusions then, can they? If you want a deep dive into the literature, I covered it for my hormone therapy guide for The Vajenda, and I will be posting a summary hopefully tomorrow. I will put a link to my Vajenda piece in my stories.

What do you think? Is there more money in menopause than liver cleansing? A while back I read a study abstract that looked at the top 10 selling “liver cleansing” supplements on Amazon. The authors calculated sales for one month. And it was a sh*t ton of money. So I decided to do just that for menopause supplements, with some help from @drjenadjacent who knows a thing or two about tech and marketing. Folks, the menopause gold rush is real: the top ten menopause supplements generated $3,363,997 in sales on Amazon in one month, easily beating $2,442,963 for “liver cleansing.” In just one month, 93,543 units were sold, which is fewer units than for “liver cleansing,” but there was more money to be made due to higher pricing for menopause. Yeah pink tax! The unit prices for menopause ranged from $13.99 (Estroven Sleep Cool for Menopause Relief) to $89.00 (BB Company Slim Gut Bundle). The average price per unit for the top 10 menopause supplements was $36.89 vs. $26.45 for the liver supplements. Remember, this is just from one month from Amazon by unit volume; this does not include other online retailers, brick-and-mortar stores, and all of those suppliers with their own Shopify or e-commerce site. Oh, and there is no quality science to support any of the top 10 products and many contain concerning ingredients. I wrote about it for The Vajenda and also about what Amazon requiring third-party testing for supplements means. I think it’s fair to point out that many providers and influencers who profit from supplements for menopause, either via their own products or by ads and discount codes, bemoan the lack of research in menopause. And while that point is valid, the answer isn’t to join the patriarchy and sell understudied, potentially harmful products to women. The right answer, when faced with a lack of research, is to raise the money and do the f*cking research. It’s clear that almost everyone selling these products is making enough profit to do this. It’s a choice to not do the science that women deserve.

The 2022 @menopause_society Guidelines for hormone therapy hold. Menoinfluencers who criticize the guidelines as being out of date are either unaware of the guidelines and the data are or cherry picking. I said what I said. I urge people to reference what they hear online with the Menopause Society Guidelines. There are plenty of good reasons to prescribe hormone therapy that we don’t need to invent more! And scaring women by creating fear of missing out is cruel. Also, a lot of frustration at this meeting about disinformation online from menopause influencers. It came up in many question sessions. How do people profit by going against the guidelines? Claiming they “know more” and creating FOMO to drive social media engagement, as a larger following can be used to make money from ads or selling supplements. This technique also drives people to extremely expensive concierge practices under the illusion that these health care professionals “know more.” Over prescribing testosterone because it requires blood levels, which are a way to make money. Recommending compounded hormones, which provide the illusion of customization but are in reality less safe. Blood levels are also typically done here, also a way to make money. Making money from pellets. Increasing revenue is literally a selling point from these businesses!
I’ve been asked a few times about antimicrobial underwear and if it can really reduce yeast infections or bacterial vaginosis or if it is just better overall. These products are unstudied for this reason, so some companies may use slippery language so you draw the conclusion that they are healthier. I wrote about these products for The Vajenda and about some of the studies that look at underwear and infections. Basically, these products seem like solutions looking for a problem. Whether they are less stinky in a gym bag is not a medical issue. With underwear, the basics are you want something that is comfortable and, if you sweat, wicks moisture from the skin.
